繼上次提及社區照顧劵後，今次延續施政報告針對居家安老的措施，加強離院支援計劃亦是其中之一針對重點。醫院管理局的離院長者綜合支援計劃會擴大，明年第三季起每年受惠人數增加三分之一至45,000人，其中可獲轉介至家居照顧服務的人數由約9,000人增至11,000人，以支援更多出院長者居家康復。 離院支援計劃簡言之是為一些即使出院，在自理上仍出現困難的長者，提供短期的照顧支援。
I. 60歲或以上；以及
II. 因病況暫時失去自理能力，經醫院管理局（醫管局）的醫護人員評估為有過渡期護理及支援需要（即出院後需要臨時院舍住宿照顧及／或社區照顧及支援服務）
服務計劃提供時間有限期，一般不超過四個月。
