[Sunyu Taiwan] Home Service of the Lao Wu Lao Foundation

In line with the Government's implementation of Long-term Care 2.0, the Elderly Commission will combine various social resources to help the elderly to realize ageing in place, and to provide diversified continuous and integrated care services ranging from supportive family, home-based, community-based to residential care.

 

Clientele:

1. Those who are eligible for subsidies according to the regulations of the 10-year long-term care plan, and have been assessed by the long-term care management center of the local county or city government to be incapable of performing activities of daily living (ADLs) or instrumental activities of daily living (IADLs), and are disabled and in need of assistance from others.

2. seniors over 65 with only IADLs who are disabled and live alone.

3. Physically and mentally challenged persons under the age of 50 who are not recipients of the Long-Term Care 10-Year Plan, and who have been assessed by the local county and municipal government Long-Term Care Management Centers as needing assistance with daily living functions.

4. For the rest of the persons who are assessed by the social worker to be in need of assistance in daily living functions, services will be provided according to the social worker's assessment results.

 

Service Description:

The following services are provided by professional caregivers

1. Household and daily life care services: including washing and repairing of laundry, cleaning of the living environment in the main living space of the client, clerical services, food and beverage services, accompanying or shopping for daily necessities, accompanying to medical appointments or contacting medical institutions (organizations), housekeeping services and other related home services.

2. Physical care services: including assistance with bathing, dressing, oral hygiene, eating, medication, turning, back patting, limb and joint movement, getting in and out of bed, accompanying for walks, exercise, assistance in the use of daily living aids and other services.

3. Other services: Chatting and emotional relief, telephone greeting and other related services.

 

Apply for a pipeline:

1. Apply by phone to the Social Welfare Bureau, Long-term Care Management Center or Home Help Center in each district.

2. Apply in person at the Social Welfare Bureau, Long-term Care Management Centers, and Home Care Centers in each district.

 

Application Process:

1. Application by the person or a family member

(1) Application at the Acceptance Counter ‧New Taipei City Public Offices ‧Health Centers ‧Regional Social Welfare Service Centers ‧Discharge Preparation Service at Hospitals

(2) Fax the application form to the branch office of the Cheung Chau Center.

(3) Online Application for E-Services in Hong Kong

2. Acceptance of cases by long-term care management centers

3. Care Manager's visit to assess

4. Development/approval of long-term care grants and programs

5. Linking Services

6. Periodic assessment, review

 

Service charge:

1. Low-income households: fully subsidized by the Government and partially exempted from the burden of the Government

2. Medium and low income: Benefit amount X Partial burden of 5%.

3. General account: Benefit amount X 16% partial burden

(Depending on the level of disability 10,020~36,180NTD/month)
