[舜禹香港] 香港藥學服務基金長者使用藥物支援及教育計劃介紹

香港藥學服務基金長者使用藥物支援及教育計劃於2022年4月推行，承蒙香港賽馬會慈善信託基金的捐助，開始為期三年的服務計劃，提供藥物有關服務予社區長者。透過藥物管理及教育服務，改善長者用藥情況及治療成效，至今已有逾500人受惠。

Eligibility:

60至69歲長者。

Service Description:

🔹 針對65歲或以上、患有2種或以上慢性疾病、獲處方5種或以上但藥物曾有變更的長者，提供為期3年的一對一服務。專業藥劑師會與長者會面及作評估，為他們提供藥物諮詢及整理服務，包括：建立完整病歷和藥物清單、協助將藥物按用藥時間放入藥盒等，並適時跟進有需要的個案。

🔹 由藥劑師以講座形式，專為60至69歲長者而設的主題藥物講座及基礎藥物管理證書課程，令長者掌握基本用藥知識，輕鬆地管理自己的健康，達到「自己健康，自己管理」。
