To encourage early prevention and management of chronic diseases, the Government has launched the "Chronic Disease Management Programme" (CDMP) to provide an incentive in the form of partial subsidy for people who are not yet aware of their chronic diseases such as hypertension and diabetes, or who have symptoms of these diseases, to encourage them to understand their own health conditions through screening at an early stage, with a view to achieving the goal of "early detection and treatment". Implementation date: Planned to be implemented in mid-November 2023 Target: All Hong Kong residents aged 45 or above who have not been diagnosed with diabetes mellitus or hypertension are eligible to participate. Service Content: Participants can receive screening for diabetes mellitus and hypertension in the private healthcare market, consultation and laboratory tests by doctors, prescription of medication according to their health conditions, and follow-up by nurse clinics and allied health services. Costs: 🔹 At this screening stage, program participants pay a one-time fee of $120 flat, which covers the cost of all screening appointments, related labs and tests 🔹 If participants are diagnosed with high blood glucose, diabetes, or hypertension, they receive a government subsidy for long-term follow up with a family doctor of their choice. 🔹 People with high blood glucose are entitled to a maximum of four subsidized consultations per year, while those with diabetes mellitus or hypertension are entitled to a maximum of six subsidized consultations per year. During the treatment stage, the Government will subsidize a consultation fee of $166 per visit, and the co-payment for each visit will be determined by the family doctor. 🔹 Participants are not required to pay for drugs covered by the doctor's prescription list and/or drugs for episodic illnesses for a maximum of three days.
