🟣[Sunyu HK] HA's Glaucoma Treatment Collaboration Project
HA's Glaucoma Treatment Collaboration Programme (GTCP) provides an option for stabilized glaucoma patients currently receiving treatment at ophthalmology specialist outpatient clinics (SOPCs) to receive private specialist medical care in the community and to choose a participating private ophthalmologist to follow up on their glaucoma condition. Target beneficiaries: Glaucoma patients who are currently receiving treatment at ophthalmology specialist outpatient clinics (SOPCs) and are in stable condition. Participation Methods: HA will send invitation letters to eligible patients in batches, and patients do not need to apply on their own. Selection Criteria: HA will invite eligible patients to participate in the Program in phases according to the date of their first glaucoma consultation at the ophthalmology specialist outpatient clinics in order of priority. 🔹 Stable condition 🔹 1 year or more from the date of follow-up consultation 🔹 Follow-up consultation at least once every 16 weeks in the past year Program Description: Each patient is entitled to up to 4 subsidized consultations per year, including specific glaucoma medications, and up to 1 visual field examination and 1 optical coherence tomography (OCT) scanning service per year by a private ophthalmologist. Fees: Participating patients are only required to pay the equivalent of HA's specialist outpatient services per visit - currently $80 per visit, $15 per drug and $80 per examination. Remarks: Recipients of Comprehensive Social Security Assistance, holders of Certificates of Full or Partial Medical Fee Waivers or recipients of Old Age Living Allowance aged 75 or above are entitled to the same fee waiver arrangements as those for HA services. Hong Kong Adventist Hospital Charitable Foundation - Tsuen Wan Council and Hong Kong Adventist Hospital - Tsuen Wan announced on 24 October 2023 the launch of the "2023-24 Charitable Heart Disease Project", which provides full subsidy for balloon coronary angioplasty (a.k.a. "Tomobojie") to eligible patients. The program covers pre-operative consultations and required tests, Tomobayashi surgery and post-operative consultations on a first-come-first-served basis. 🟣 [Sunyu Hong Kong] Support Service for Hoarders
In Hong Kong, there is always a shortage of messy houses, always refusing to throw away old things and constantly scavenging to bring home more waste, eventually leading to a messy home, poor sanitation and finally evolved into a "garbage house". Psychiatrists have pointed out that these behaviors may be related to hoarding. This article provides information on how to support hoarders. Hoarding is caused by a knot in the heart: Most of the hoarders have unresolved knots in their hearts due to prolonged loneliness, anxiety, and restlessness. The antidote to hoarding is to deal with emotions and internal needs. It is not advisable to help the hoarder to enforce the house on his own without professional assistance, as the environment may not be safe, and gloves and insect repellents are needed. If you only enforce the house without facing the emotional problems in your heart, you will be back to your original form very soon. Home Care Outreach Service "Casework A case management team comprising social workers, psychologists and health care professionals will provide counseling services to persons affected by hoarding behaviors through a multi-disciplinary approach. A case management group comprising social workers, psychologists and healthcare professionals provides counseling services to people affected by hoarding behaviors. A "Hoarding" and "Completion" group A group approach is adopted to support people who have been affected by hoarding behavior and to build up mutual support networks. A "Hoarding" and "Completion" cohort group The group uses a group model to provide support to people who have experienced hoarding behavior and to build a mutual support network. "Home" and "Friends" Volunteer Service Program To organize street sleepers or community members to provide hoarding victims with hoarding services such as hoarding disposal and household pick-up as far as practicable. "National Public Education Program To utilize the power of social media to raise public awareness of hoarding in an innovative way. Provide the public with knowledge and related community resources. Application: 1. Applicants must be referred by a social worker. 2. 2. The referrer must confirm the applicant's wishes and ensure that the applicant is willing to accept the services provided by the program, and must fill out the application form accurately. (Application forms must be obtained by contacting The Salvation Army.) Service Approval: 1. Program staff will confirm the referral verbally or in writing within 21 working days of receipt of the application form. 2. 2. after the first interview and assessment by the Program Manager, service approval will be made according to the applicant's condition and needs. 3. If it is not possible to provide appropriate services to the applicant, the responsible person of the program will provide the applicant with information on social welfare services verbally or in writing. Exit from the service: 1. Service target or referral agreement is met. 2. The applicant has been out of contact for more than three months. 3. Applicant passes away 4. The applicant is uncooperative and the social worker is unable to provide services. 5. 🟣 [Sunyu Hong Kong] Elderly Health Care Voucher Reward Pilot Program
The Government's "Elderly Health Care Voucher Reward Scheme" will be launched on November 13 for a period of three years. Eligible elders who have used $1,000 or more health care vouchers for specific primary health care services in the same year will automatically be issued $500 reward vouchers, and this article organizes the information of the scheme. Eligibility: Elderly people are required to use $1,000 or more of health care vouchers in the same year for specific primary health care services, such as disease prevention and health care management, and will automatically be issued $500 bonus health care vouchers. Specific primary care services are as follows: 1️⃣ Disease prevention, follow-up or monitoring services for chronic diseases provided by Chinese and Western medicine practitioners and dentists participating in the health care voucher scheme, such as health assessments, medical check-ups, screenings, vaccinations, management of chronic diseases, dental check-ups, cleanings, extractions, fillings, and so on. 2️⃣ Chronic Disease Management Pilot Program (CDMP) hypertension and diabetes screening, and treatment phase services. 3️⃣ Individualized services and community rehabilitation programmes at district health centers or district health posts 4️⃣ Outpatient services for disease prevention, follow-up or monitoring of chronic diseases at the 11 designated clinics of the University of Hong Kong Shenzhen Hospital (HKU-SZH) and its health service centers Scope of use of the rewards: The rewards can only be used for the four specific services mentioned in the title above, excluding optometric services. Expiry date of the rewards: Each year's rewards are valid until December 31 of the following year, after which they will be forfeited. Unused rewards cannot be accumulated and the amount accumulated in the following year will be recalculated. The rewards will not be counted towards the $8,000 voucher accumulation limit. Other FAQ: 1️⃣ Can I use the rewards immediately? A: The rewards can be used the next time you receive a specific service. The system will automatically deduct the rewards (if any) before deducting the remaining balance in your voucher account. 2️⃣ How many times can I receive rewards each year? A: One time, the total amount of vouchers for eligible elderly is $2500 for the year. 3️⃣ Can I share my rewards with my spouse? A: No. 4️⃣ If I use my spouse's vouchers to pay for my healthcare, which spouse will receive the rewards? A: If the elderly person's voucher account has been depleted and the spouse's vouchers are used to pay for specific primary healthcare services, the utilization will be credited to the accumulated amount of the elderly person (i.e. the recipient of the service, not his/her spouse) for the specific primary healthcare purpose. 5️⃣ Will the use of rewards to pay for services be counted as the cumulative amount of rewards received? A: No, it will not. That is to say, if you are awarded in this year and want to receive the reward in the next year, you need to use $1,500 worth of services and pay with $1,000 health care vouchers and $500 rewards. 🟣[Sunyu HK] Chronic Disease Co-management Pilot Project
To encourage early prevention and management of chronic diseases, the Government has launched the "Chronic Disease Management Programme" (CDMP) to provide an incentive in the form of partial subsidy for people who are not yet aware of their chronic diseases such as hypertension and diabetes, or who have symptoms of these diseases, to encourage them to understand their own health conditions through screening at an early stage, with a view to achieving the goal of "early detection and treatment". Implementation date: Planned to be implemented in mid-November 2023 Target: All Hong Kong residents aged 45 or above who have not been diagnosed with diabetes mellitus or hypertension are eligible to participate. Service Content: Participants can receive screening for diabetes mellitus and hypertension in the private healthcare market, consultation and laboratory tests by doctors, prescription of medication according to their health conditions, and follow-up by nurse clinics and allied health services. Costs: 🔹 At this screening stage, program participants pay a one-time fee of $120 flat, which covers the cost of all screening appointments, related labs and tests 🔹 If participants are diagnosed with high blood glucose, diabetes, or hypertension, they receive a government subsidy for long-term follow up with a family doctor of their choice. 🔹 People with high blood glucose are entitled to a maximum of four subsidized consultations per year, while those with diabetes mellitus or hypertension are entitled to a maximum of six subsidized consultations per year. During the treatment stage, the Government will subsidize a consultation fee of $166 per visit, and the co-payment for each visit will be determined by the family doctor. 🔹 Participants are not required to pay for drugs covered by the doctor's prescription list and/or drugs for episodic illnesses for a maximum of three days.
